The Lake Forest High School Wellness Center provides physicals to play sports and will have a limited number of openings during the summer for students in grades 9-12 who plan to participate in a fall sport.
Before this service can be utilized, a parent or guardian must complete a five-page Wellness Center enrollment packet and Delaware Interscholastic Athletic Association (DIAA) Sports Packet. Please fill out forms completely and return them to the center. Once the completed packets are returned, an appointment will be scheduled. Sports physicals completed after April 1 (incoming ninth-graders can receive physicals beginning July) will be valid for the 2012-‘13 school year. The fall sports season begins Aug. 15.
Students are strongly encouraged to schedule physical appointments in April and May.
The Wellness Center is normally open when school is in session from 7 a.m. to 3 p.m.; however, hours are reduced in the summer.
